fix: match parsing card header to Figma page pill and breadcrumb

Summary

• Restyle the Parsed Results card header to the Figma spec: a Page N pill plus a Root / current section breadcrumb.
• Drop the extra type icon and type chips so the header matches Knowhere (Copy) / 4120:43011.
• Follow-up to feat: show type, page, section, and entities on parsing cards #160, which merged before this header match landed.

Test plan

• Open Parsed Results → List and confirm a page card header is Page N + Root / …, not a type icon row.
• Confirm the pill is white with a light gray border and the last breadcrumb segment is darker than Root.
• Confirm image/text cards still show a lead pill (type or page) and a section breadcrumb when a path exists.
