Fix DX11 hooks and offset scanner

Summary

• align all __thiscall detours with __fastcall convention and guard trampolines
• replace unsafe hex parsing and tighten offset scanner to avoid mixed addresses
• add resilience around Present hooks and enable scanning only when all signatures resolve
