codex 0.154 asks before any MCP tool not marked read-only through mcpServer/elicitation/request, and the loop answered only */requestApproval, so the thread sat in waitingOnApproval and every MCP write stalled while reads went through (found on hosted, 2026-09-30). Permissions are granted in their own shape, approvals accepted, codex's own elicitations accepted, an MCP server's question declined, user input answered empty, and anything else refused with a JSON-RPC error at once.
