[Consolidation] Org track 17/18/19: merge into a single chapter (-430 lines) #1906

Problem

Chapters 17 (organisational change, 105 lines), 18 (team structure, 279 lines) and 19 (management as code, 300 lines) say the same thing. The review identifies roughly 70% overlap.

Concrete duplicates

Proposed new structure

A single chapter "Organisation, teams and leadership" (~250 lines):

1. Transformation and team structure (~80 lines, drawn from 17 + 18)
2. Competencies and culture (~70 lines, drawn from 17 + 18)
3. Management as Code: leadership artefacts in Git (~100 lines, drawn from 19, focused on what is GENUINELY unique — budgeting-as-code, OKR-as-code, policy-as-code for leadership decisions)

Major cuts within the current material

• 19_management_as_code.md:72–251 — DELETE 180 lines on GitHub Issues/Discussions for management; it is a drawn-out ode to GitHub features already explained in chs. 3, 4, 11 and 22.
• 18_team_structure.md:62–139 — DELETE the 12-week YAML training programme (no organisation implements this literally).
• 18_team_structure.md:153–189 — DELETE "team-structure.yaml" with fantasy role figures.
• 18_team_structure.md:199–261 — DELETE 50 lines of CoP template which belongs in Confluence, not in a book.
• 19_management_as_code.md:286–290 — DELETE "smart contracts for management rules" speculation.
• 19_management_as_code.md:286–287 — DELETE the fabricated anonymous case study.

Consequences

• Update docs/book_index.json (3 → 1 chapter).
• Update part_e_*.md (Organisation & Leadership).
• Update references in 33_references.md.

Expected saving

~430 lines (63% of the track) — from 684 down to ~250 lines.
