[Cross-cutting] Eliminate cross-chapter duplication (OPA, "assure once", AaC vs IaC, EU mantra) #1899
Description
Problem
The review identified a number of concepts introduced or repeated in 3–5 different chapters. This is the single largest cause of the book's bloat.

Principle going forward
A tool, concept or source is presented in exactly one chapter. Other chapters refer to it with a single sentence: "OPA is introduced in ch. 10."
Expected saving
~150 lines scattered across the book's chapters. The larger gain is in clarity and navigability than in line count.
