A repository-wide concurrency group lets GitHub cancel a queued run when another pull request queues behind it, which left the required mutation result check missing.

The mutation workflow's concurrency group was shared by every ref, so a queued run for one pull request was cancelled when another queued behind it, and the required mutation result check never appeared on the cancelled one. Scopes the group to the ref.
