docs(storage): list TrueNAS cleanup for democratic-csi

Follow-up to #39.

• docs/TRUENAS_CSI_MIGRATION.md: replace the stale "set install_democratic_csi = false and apply" cleanup steps with the current state (nothing left on any cluster) and a manual TrueNAS checklist: SAS/RKE2 + SAS/RKE2/pvc-* datasets, SAS/RKE2-snapshots, NFS shares under /mnt/SAS/RKE2/, and revoking the democratic-csi API key (TrueNAS CSI uses its own truenas_csi_api_key → truenas-api-credentials). TrueNAS CSI volumes live at SAS/pvc-*, so the SAS/RKE2 tree is unused by it.
• CHANGELOG.md: note that the stale democratic_csi_config root output was dropped from Terraform state (state backed up first; state edit only, resources byte-identical, no apply).

Docs only; does not touch UPGRADE_PLAN.md / OPS_NOTES.md / RKE2_CERT_ROTATION.md.
