[Bug] Remove Include option from 16-19 student data amendment options

Overview
The options displayed on the What student data would you like to change? page within the 16-19 checking window contains the Include option which needs to be removed.

Steps to reproduce

1. Navigate to DEV/QA and sign in.
2. Enter the 16-19 checking window and at the bottom of the Check your student data page select Request an amendment to student data and click Continue.
3. Note that three options display including the Include option.

Expected result
Only two options are displayed: Merge duplicate student records and Remove a student from data.
