Restore entity-specific identity bindings
This unrelated governance edit changes an entity-specific alternative into a requirement that every identity contain a snapshot revision, model version, request/result scope, and label version. The cross-file counterpart in docs/topic-intelligence/DATA_MODEL.md:155-165 defines distinct tuples—for example, document snapshots have no model or label fields, while model artifacts have no request or label fields—so the new wording contradicts the canonical data model and would cause future reviews to demand irrelevant fields.
