chore(stack): combine essay title and row-header hardening

Temporary branch-reconciliation PR. Merge #1007 exact head e2d5e0ebfe11d9b842a88f4773dbad3d4c577ccb into #997 exact branch head 81f0985533b17cc0db51472174c8f2a66bce155d.

The shared production paths are validation_report_html.py and calibration_report_html.py, but the changes are independent and complementary: #1007 adds exact-built-in title admission before .strip()/escaping, while #997 adds row_header_column=0 to the validated table call sites. Preserve #1007's score-report title change, hostile-title regression, and title fragment; preserve #997's row-header regression, WCAG doctoring, and row-header fragment.

No score/calibration/validation arithmetic changes. This PR exists only to create one non-force combined source branch; do not merge it to main as a standalone landing unit.
