When an explicitly untrusted ingest uses a configured Extractor, this branch accepts each ExtractedFact.metadata and later merges it over the ingress metadata via metadata={**base_metadata, **fact_own}. Because the protocol permits arbitrary fact metadata, an extractor that returns provenance.trusted=true can replace the service-owned untrusted provenance, making externally ingested content prompt-eligible and able to resolve against trusted memories. Preserve authority fields such as provenance and quarantine from the ingress envelope, and whitelist only the structured extraction fields the extractor may contribute.

Include source titles in grounded support
When a packed memory is found only by its title, this recomputation discards that title and scores only the body excerpt. For example, querying OAUTH_TOKEN_ROTATION retrieves a memory titled OAUTH_TOKEN_ROTATION with body Every 30 days and reports absolute_support=0.5, but grounded recall recomputes support as 0.0 and incorrectly abstains; include the packed title in the support text, as the recall and adaptive-context paths now do.

Delete quarantined vectors from sqlite-vec too
When --apply rescans a database that has used the sqlite-vec backend, this removes only the portable mem_vectors copy while leaving the same quarantined ID in mem_vec_ann. Those retired entries remain in every ANN search; because SqliteVecVectorIndex.search() must widen past temporally invisible hits, quarantining a large legacy import can force repeated searches toward full-index scans indefinitely. Remove the ID from both persistent vector mirrors.
