Adds .gitleaksignore. The CI secrets scan failed on the planted fake key of the invented test site (tests/fixtures/code-site/lib/keys.ts, tests/run_all.py), which the tests use to prove no skill output repeats it. The file lists those two findings by fingerprint, for the git scan and the folder scan.

An earlier commit on this branch added a DECISIONS.md line; it is reverted, so the squashed change is the ignore file only.

Checks: gitleaks git --log-opts=HEAD and gitleaks dir find no leaks; bash .github/scripts/check-standard.sh and the canonical check with this title and branch pass.

The invented test site carries a fake key on purpose; tests/run_all.py checks
that no skill output repeats it. The fingerprints cover the git and the folder scan.
