docs: client support table (verified × spend-dialog × install) - #121

Replaces the collapsed 'Other clients' block with one table per client: status (Verified = we ran the published 0.43.0 on that client today and saw its tools listed — Claude Code 2.1.251, Codex CLI 0.142.5; OpenClaw is in daily use from a local build; the rest are Documented from the client's own docs), whether the client renders the spend-confirmation dialog (from docs/spend-confirmation.md's verified matrix), and the one-line install. Adds VS Code (code --add-mcp) and Gemini CLI (gemini mcp add -s user) rows with syntax taken from their docs. JSON snippets for Claude Desktop, Cursor and Windsurf stay, collapsed, with the file paths in one table.
