Fix Windows launcher recovery when Codex is already running

Summary

• make the Windows single-instance guards resilient to stale loopback ports
• retry launcher injection by restarting the packaged Codex app when CDP args are swallowed by an existing instance
• add a launcher regression test for the Windows restart-and-retry flow

Testing

• cargo test -p codex-plus-core launch_lifecycle_restarts_codex_once_when_injection_retry_is_prepared -- --nocapture
