feat: consolidate installed projects under .ai-workflow/ - #17

Installed projects previously scattered core/, VERSION and adapter
notes across the target root. Everything APW-internal now lives in
.ai-workflow/ so the project root only shows the user's own files,
the AI entry points (AGENTS.md, CLAUDE.md) and the tool
dot-directories that must stay at the root:

adapters/ platform notes
- layout.js: content modes (core = this repo, runtime = installed
projects), path helpers and the core/ -> .ai-workflow/runtime/
markdown rewrite (idempotent)
- install: copy core/ to runtime with rewritten references, VERSION
into .ai-workflow/, adapter notes into .ai-workflow/adapters/
- adapters: render/check/sync are content-mode aware; legacy installs
that still have core/ at the root keep validating unchanged
- workflow config: .ai-workflow/project.yaml primary, .apw/ legacy;
custom skills resolve from .ai-workflow/skills/ first
- validate: structure, templates, schemas, agents and skill lookup
follow the detected layout
- migrate: one-shot upgrade moves core/, VERSION, .apw/ and adapter
notes into .ai-workflow/ and rewrites path references; source
repositories are never touched
- tests: 30 passing, including a legacy-to-new migration end-to-end
- README/README.zh-CN: diagram, walkthrough, config and FAQ updated
