[Bug]: Cannot log in with ChatGPT Plus sub

Bug Description

Trying to log in with Codex to use my ChatGPT plus sub, I get a "configured successfully" and a bearer token error instantly:

● [18:13:43] Codex configured successfully
● [18:13:43] ERROR: Bearer token is required in API key field

Both the authorization code and device flow produce this error for me. Logging into codex agent using the same account works (and the model responds). It then closes the agent, and upon trying to re-open it and select codex again (it prompts me to on open) the error repeats. If I dismiss this, any prompt results in ERROR: No default provider and model configured..
I have a ChatGPT Plus ($20) sub, if that matters.

Steps to Reproduce

1. :login -> codex -> pick any flow

Expected Behavior

I expect to be able to use GPT models with my chatgpt subscription.

Actual Behavior

Logging in with a chatgpt subscription throws an error.
