Errors in retrieve: Package and GlobalPicklistValue

All metadata types retrieve correctly except these two.

Package retrieval gives the following state-detail:

'INVALID_ID_FIELD: Metadata#getAllNames :: \\'Package\\' has null entity id; will raise a Null Pointer Exception in Udd.getEntityCommon(<entity id>)'

There may be a good reason for this: perhaps we need to list packages explicitly? Is this related to the unimplemented packageNames parameter mentioned in #8 ?

GlobalPicklistValue:

state_fail contains 'UNKNOWN_EXCEPTION: An unexpected error occurred. Please include this ErrorId if you contact support: 182602367-44274 (571045392)'
