Define a unified invocable skill descriptor contract

One descriptor represents a skill from every supported source.

Requirements

• Reuse the current invocable and argument-hint metadata.
• Include the canonical name, description, source, and audience visibility.
• Represent local, skill-server, and MCP prompt sources.
• Keep MCP prompt argument schemas in the MCP adapter.
• Do not require file paths for remote skills.
• Preserve compatibility for current file skills.
• Define a stable client contract for the daemon API.
• Keep model invocation and user invocation as separate flags.
