Ability to handle multiple browser windows and windows opening/closing at anytime

• Cache the new markdown content on every PUT request
• On every GET request, initiate a new websocket and send the latest cached markdown so the the browser gets the latest state

Among other things, will solve instant-markdown/vim-instant-markdown#92
