Subject: [PATCH] fix(templates): drop KubeStellar leftovers from issue forms

The bug report form asked for kubestellar-snapshot.sh output and linked
a 404 troubleshooting page; the doc issue form linked a nonexistent
hivecommons/kubestellar README. Ask for page URL and browser/OS instead,
point doc reporters to README "How content is sourced", and replace the
kubectl kcp epic placeholder.
