Path should be references/planner_instructions.md instead of ../planner_instructions.md for both:

1.  **Resource Initialization (MANDATORY):** Before formulating any plan, you
    MUST read instructions from `../planner_instructions.md`. Do not
    proceed until these instructions are in your active context.

And

7.  **Review Planner Principles:** Crosscheck planning directives in
    [planner_instructions.md](../planner_instructions.md).

Same fix applies to references to ../terraform_validator_instructions.md and ../generator_instructions.md
