Set up Claude Code automatic PR reviews

Add GitHub Actions workflows so Claude automatically reviews pull requests into dev, plus an interactive workflow for requesting Claude on issues and PR comments (needed for fork PRs, which cannot see repo secrets). Auth via the CLAUDE_CODE_OAUTH_TOKEN repo secret.

Done. Automatic Claude reviews are live for PRs into dev (claude-code-review.yml), and @claude mentions work on issues and PRs (claude.yml), including fork PRs where the automatic path cannot run. Both workflows are on main and dev. Verified end to end on PR #842.
