Namespace values

Instead of stuff like 

* "mem_total" => "value"
* "mem_used" => "value"

Let's do 
* "mem" => "total"
* "mem" => "used"

I'd also like to test a telegraf mode to write to "telegraf" database and use the same naming scheme.  Hopefully hosts would automatically show up in the Chronograf host list.
