Allow opt-out updating agent instructions file(s)

If I use other agent instructions e.g. in CLAUDE.md file the extension is always appending additional instructions.
Is it possible to provide a setting so that we can opt-out on automatically appending agent instructions, if it has been modified once by the user?

Note: As it is not the same requirement / issue statement as in #148 I'll raised this new issue.

Will add a new setting sn-scriptsync.agentInstructions.autoUpdate (default on). Turn it off and the extension stops adding/refreshing its managed block inside your own CLAUDE.md / AGENTS.md / .cursorrules / etc.

It still keeps agentinstructions.md and the agentrules/skills/ folder up to date, so you can reference them on demand yourself (e.g. @agentinstructions.md) without anything being written into your instruction files.

Making your agent pick up an update: after an extension update, tell your agent to re-read them, e.g. "re-read agentinstructions.md and the agentrules/skills, they may have changed." With tools that import the file (Claude's @agentinstructions.md, Cursor rules) a fresh session picks up the latest automatically.
