V32 Gate 5: Prove settlement failure states

Adds the Gate 5 source-safe ledger/BTD settlement failure-state proof artifact, generator, closure checker, and focused BTD test coverage for BTC fee phases, blocked readiness, BTD receipts, source-to-shares conservation, projection drift repair, paid unlock, and PR delivery readback.

Updates the V32 proof matrix, deterministic replay artifacts, gate-quality workflow, package scripts, roadmap, and V32 specification family so the gate is documented and CI-enforced.

Validated with V32 Gate 1-5 checks, V32 spec-family/canonical/posture checks, BTD typecheck, focused Gate 5 Jest coverage, full BTD Jest coverage, diff hygiene, and secret-pattern diff scan.
