V41 Gate 9: Close Prompt Program Promotion Readiness

Adds the package-backed V41 prompt-program promotion readiness report and generated source-safe artifact.

Wires V41 Gate 9 into package exports, protocol tests, gate/canon quality workflows, the V41 canonical promotion workflow, promotion scripts, and proven generation.

Updates V41 specification, roadmap, README, and protocol README posture for active V41 / draft V42 promotion readiness.

Regenerates dependent V41 artifacts after closing Gate 9 roadmap posture predicates.

Validated with V41 gate checks 1-9, promotion dry run, protocol tests, protocol typecheck, changed-file secret scan, and git diff checks.
