V45 Gate 17: Rehearse source-safe end-to-end loop

Add the V45SourceSafeEndToEndRehearsal protocol artifact, generator, checker, and source-safe operator rehearsal script across local deterministic, staging-testnet credentialed, and value-bearing mainnet-blocked lanes.

Bind the rehearsal into V45 specs, proof-family generated artifacts, workflows, package exports, protocol tests, README documentation, and the roadmap. The rehearsal fails closed into repair state for missing or contradictory evidence and withholds source-bearing, prompt, provider, credential, wallet, private settlement, and live-log payload classes.
