Look at changes if staged changes is empty

Hi there, first of all, thanks for the great simple extension. Was struggling with the other top 2

Sometimes I don't stage stuff and straight commit, I think the default auto behavior should be just look at staged if staged present, else look at all changes to generate the commit message

As a default feature we likely won't enable such behavior because it is counter-intuitive to the idiomatic SCM/git process for committing changes. However we can add a setting/flag to enable such behavior and document that!
