Subject: [PATCH] Add the NOTICE that Apache-2.0 asks for

The repository ships Apache-2.0 and had no NOTICE. Section 4(d) of the
license makes NOTICE the place where attribution travels with the code, and
without it a redistributor has nothing to carry forward.

It also records what LICENSE alone cannot: ferramental/ was vendored from
the alicerce project, under the same license, and adr/0002 holds the
decision. Anyone reading this repository now learns where that tree came
from without having to guess.

Found by rebar-check (github.com/Navesz/rebar), rule `notice`, which pairs
"the LICENSE says Apache" with "a NOTICE exists".
