Build milestones configuration form (Step 3)

User Story:
As a creator, I want to define funding milestones so that donors know how funds will be released progressively.
Acceptance Criteria:

• Add up to 5 milestones
• Each milestone: title, target amount, description, expected completion date
• Milestones must be in ascending order
• Final milestone = total goal amount
• Drag-and-drop reordering
